Meet Your Care Team

As a patient at our Center, you or your loved one are going to be working with a variety of health care professionals with the right knowledge to help you reach your goals.

Nurses

Every patient that enters our Center will be cared for by a registered professional nurse in collaboration with other nurses and clinical care professionals on the floor. Your nurse will work with you to plan your care and will also work closely with other members of the health care team (physicians, discharge planners, therapists, etc.) throughout your stay. He or she will help you understand the nature of your illness, its treatment, and how to care for yourself after discharge. If you have any questions or concerns during your stay or after discharge, feel free to discuss them with your nurse. Our Unit Managers and Director of Nursing will also be happy to work with you to address any concerns regarding nursing care.

Certified Nursing Assistants (CNAs)

Our Certified Nursing Assistants (CNAs) are part of your clinical care team. They work closely with you on daily tasks that may be difficult on your own, such as bathing and dressing. This team of compassionate individuals work very closely with the nurses to make sure you are staying on track with your care plan and getting you ready to start each and every day.

Therapists

Our therapy team is comprised of physical therapists (PT), occupational therapists (OT), speech therapists (SLP) and other therapy professionals all with the goal of helping you get back to health. Through our flagship LifeWorks Rehab product, this interdisciplinary team of therapy professionals works with you or your loved one from day one, on a personalized plan of surgery or illness recovery. He or she will help you set goals and show you ways to continue progress after discharge. If you have any questions along the way, our Director of Rehab is always willing to work with you to make your experience the best it can be.

Discharge Planners

We recognize that recovering from an illness or surgery can be a stressful experience for both patients and families. To help you and your family cope with these stresses, a discharge planner is available as an integral member of the health care team. Our discharge planners provide a variety of services that range from individual, family or group counseling to finding accommodations for patients and families who are from out of town. As a member of the health care team, your discharge planner will be with you from day one, developing and assessing your needs for when you return home.
